If you are interested in referring someone to Trivium, please complete the referral inquiry form below and someone will be in touch to discuss your next steps and how we can help.
For our Long Term Support Services, there is an application to complete. During the application process, Trivium will cover the admission criteria and provide a list of required documents and forms that need to be completed. If you are currently working with a case manager, please be sure to discuss your interest in Trivium with them.
